Transitional care is an optimal setting for patients that require a regimen of skilled therapy or nursing care following an acute hospital stay. Our highly-qualified therapists and nurses are skilled and experienced in working with adults of all ages who have a variety of orthopaedic, cardiac, and neurological conditions.
